Deerfield Primary Care Services

At the Healthy Living Center, if you or your dependents (14 years and older) are enrolled in a Walgreens medical plan*, you’ll pay a $5 copay. other services like testing, labs, and diagnostic services, may result in additional fees.

The Healthy Living Center provides comprehensive patient-centered and holistic care through primary care, episodic care, physical therapy, laboratory services and wellness programs using an integrated approach. Physicians, pharmacists, nurses and fitness specialists work together as a cohesive extended team at your site.

The center is equipped to handle a variety of personal medical services for adults and children age fourteen and older, including:

  • Primary care

  • Physical exams

  • Immunizations

  • Lab services

  • Disease management

  • Wellness coaching

  • Physical therapy

The Healthy Living Center gives you access to an evidence-based, integrated approach to healthcare that promotes wellness and prevention through education and engagement, as well as providing care for chronic conditions and episodic care. Whether you schedule a physical exam in advance or experience an acute care need, you can choose to use the Healthy Living Center as your family’s medical “home” or use it to supplement your current doctor’s care — it’s your choice.

Fitness Center Services

The Healthy Living Fitness Center offers onsite & virtual fitness services to all Walgreens Team Members, spouses, dependents over 18, and contractors. Whether you are on campus or working remotely, we have memberships, programs & services that will work perfectly to help you stay active and healthy.

Healthy Living Center Staff

Pamela Pauly – Health Center Manager

Pamela is a Registered Nurse, who comes to Premise with over 25 years of healthcare experience. She has held positions in the hospital setting in her nursing career, with Emergency Medicine experience educating and training many nurses and health care providers over the years. Prior to coming to Premise Health, her recent position was with the University of Chicago in the ambulatory patient care area. She is dedicated to working with incredibly gifted providers, and the dedicated Premise team members. She is enthusiastic about the design the Healthy Living Center provides for the members, providing the highest quality of care, and collaborating to support the whole person throughout their health journey. She is excited to be a member of the team at Premise Health. Pamela enjoys nature, gardening, reading, fitness, traveling, fishing, and spending time with family.


Dr. George S. Patton II, MD – Medical Director

Dr. Patton is a physician who has lived in Illinois since 2006. He grew up in Tennessee, and received his medical degree in Nashville, TN at Meharry Medical College. He received civilian and federal medical training at Mount Sinai Hospital (Chicago, IL), North Chicago Veterans Hospital (now known as Captain James A. Lovell Federal Health Care Center), and Great Lakes Naval Base from 2006-2009. Upon completion or residency training, he was certified to practice Internal Medicine. He promoted preventive medicine along with physical and mental health as the medical director of the Choose Health Wellness Center at Aon/Hewitt in Lincolnshire, IL. Before joining the Walgreens Healthy Living Center, he was a member of a small group practice in Chicago, IL. While there, he was devoted to practicing hospital, nursing    home, and outpatient care.

Outside of medicine he enjoys his family, community service, golf, and travel. He is currently a board member of IAMM (International/American Medical Mission) where he travels with a medical mission group to help underserved populations in the US and abroad. Dr. Patton enjoys being a part of Premise Health. He promotes effective, efficient, and evidence-based medicine. He likes to educate his patients and is devoted to quality care with hospitality.


S. Heather Venamore – Physician Assistant

Heather is an accomplished board-certified Physician Associate with over 25 years of successful medical practice. She completed her medical training at Cook County Hospital in Chicago and holds a bachelor's degree in Exercise and Sport Science. Originally from Colorado, Heather has focused her medical career in the Chicago area, specializing in Emergency, Occupational, and Family Medicine. Heather strongly believes in the importance of listening and effective communication in every medical encounter. She places great value on collaborative care and practices evidence-based medicine to ensure the best outcomes for her patients.

Outside of work, Heather enjoys exercising, traveling and spending quality time with her family.


Em Baker – Physical Therapist

Em Baker PT, DPT is a physical therapist & they received their Doctor of Physical Therapy degree from Northwestern University. Before coming to the Walgreens Healthy Living Center, they worked in a sports medicine clinic at Naval Station Great Lakes. Prior to attending physical therapy school, Em was in the United States Navy for 5 years as an aviation technician on F/A-18s. Outside of the center, Em enjoys spending time with their family, cooking, fitness, playing the guitar, and going to concerts. They are excited to be a part of the HLC team and for all the ways that physical therapy can help the Walgreens team members meet their goals.


Magda Jaramillo – Medical Assistant

Magda has been a Medical Assistant since 2003. She graduated from Olympia College in Skokie and did her internship in Bannockburn with a specialist group. Magda also worked with an orthopedic group in Buffalo Grove for eight years. Prior to her position here at Premise Health, she worked with the community for three years at the Health Department. She has always loved working in healthcare to help people with their medical needs.


Elena Gamo – Medical Assistant

Elena is a Certified Medical Assistant who graduated in 2009 from Westwood Collage in Chicago. She has worked in the outpatient care since then assisting in several specialties. Elena enjoys learning and keeping up with the latest in medicine through The AAMA and assist doctors provide quality patient-centered health care. She is excited to be part of the team at Premise Health assisting The Walgreens Healthy Living Center team members.


Nancy Zimmerman – Front Desk Admin

Nancy graduated from the University of South Florida with a degree in Sociology.  She worked at Baptist Hospital in the Education Department for over 10 years.  She had her own organizing business, then worked for 2 prominent Plastic Surgeons in the northern suburbs. 

Nancy has worked for over 8 years for Premise Health. She is happy to part of the Healthy Living Center and adores her co-workers and patients.


Judy Fakhouri – Worksite Pharmacy Manager

Judy graduated from the Chicago College of Pharmacy at Midwestern University in Downers Grove and has been practicing in community pharmacy with direct patient care in Illinois for over 18 years. She is an experienced pharmacist in outpatient settings focusing on patient care, medication therapy management, and immunizations.

She has been consistently drawn to patient-centered pharmacy care which led her to her current role at Premise Health. She is dedicated to positively impacting patient care and health outcomes.

 


Ron Conner – Pharmacy Technician

Ron is a certified senior pharmacy technician. He has worked in the pharmacy field for 16 years. He has been with our Premise Health site for 4 years. Ron is here to assist you in all your pharmacy needs.

 


Nicole Garcia – Massage Therapist

Nicole received her massage therapy license from the Soma Institute of Clinical Massage Therapy in Chicago, IL. Nicole has experience in Swedish massage, deep tissue, trigger point therapy, and overall therapeutic massage. She has worked 1:1 with patients in both a spa & chiropractic setting. Nicole enjoys running, hiking, playing tennis, and spending time with her loved ones in her free time.


Debbie Limer – Care Manager/Wellness Coach

Debbie is a Registered Nurse with 10 years of experience in wellness and prevention. She has been a Nationally Board-Certified Health and Wellness Coach since 2016. Debbie loves her line of work because she personally witnesses what lifestyle changes can do to an individual’s whole being. She is excited to be part of the Premise Health HLC team and work with the other providers to help members become the best version of themselves.


Caitlin Simundza – Fitness Center Manager

Caitlin received her Bachelor of Arts in Exercise and Sports Science with a concentration in Health & Wellness from Judson University. She has 8 plus years of experience in the fitness field from working as personal trainer & group exercise instructor to working in multiple corporate fitness locations. Caitlin is a NASM Certified Personal Trainer, AFAA Certified Personal Trainer, AFAA Gear (Spin) Instructor, NASM Corrective Exercise Specialist, NASM Behavioral Change Specialist and is certified in FMS level 1. Caitlin enjoys participating in Century Bike Rides, organizing and traveling.


Matt Carroll – Assistant Fitness Center Manager

 Matt received his Bachelors in Kinesiology with an emphasis in Exercise Physiology and Graduate degree in Kinesiology, Exercise Physiology and Fitness leadership specialization. He has 5 plus years of experience in the fitness field from working as strength coach for a roller derby team to personal trainer & group exercise instructor in corporate fitness. Matt received numerous certifications including: NSCA CSCS, Gray Institute 3D MAPS movement analysis, Functional Movement Screening level 1, Dynamic Variable Resistance Training level 1 & 2, Functional Range Conditioning, Mobility and Movement Specialist level 1, and Precision Nutrition level 1.

I love to read, learn, and move. I have 2 German Shepards that are balls of energy and love to go on walks with them.


Jordimar Arriaga – Fitness Specialist

Science and Health Promotion from the University of Illinois at Chicago. Jordimar holds the NASM Certified Personal Training Certification. He worked as a rehab aide at Lutheran General Hospital where he assisted physical therapists in their outpatient centers. He previously taught group exercise classes and personally trained at the Galter Life Medical Fitness Center in Chicago. Jordimar loves to cook, and listen to music, and has recently taken up sewing as a hobby!


Riley Callard – Fitness Specialist

Riley earned his Bachelor’s degree in Health Promotion with a minor in Coaching from the University of Kentucky. With over four years of experience in the fitness industry, he has worked as a performance instructor, personal trainer, and group exercise instructor. Additionally, Riley has spent two years working in corporate fitness. He holds certifications in NASM Certified Personal Training, AFAA Group Fitness Instructor, Cardiorespiratory Training for Sports Performance, Neuromuscular Stretching, Integrated Reactive Training, and Mental Toughness. In his free time, Riley enjoys playing and watching sports, spending time with friends, traveling, and watching movies.
